Week 4 Fantasy Football Injury Update and Players to Watch

By FantasyLineups | October 2, 2026
Week 4 lineup decisions are getting complicated at wide receiver and running back. Puka Nacua is moving toward a return, while DeVonta Smith, Justin Jefferson and Ladd McConkey remain concerns. Several backups could gain opportunities, but their value depends on who is cleared to play.

Puka Nacua Trending Toward a Return
Rams head coach Sean McVay expects Nacua to return against Philadelphia after missing two games with a hip/groin issue. Thursday’s official report listed him as limited, even though Nacua said he completed a full practice.

Our fantasy take: Keep Nacua in your starting plans if he is cleared, while checking for any workload restrictions. His return would also make Konata Mumpfield’s recent production harder to rely on.

DeVonta Smith and Justin Jefferson Need Backup Plans
Smith missed Thursday’s practice with a hamstring injury, and the latest report in our news feed says he is not expected to play against the Rams. That expectation is not a final inactive designation. Dallas Goedert also missed Thursday’s session with a knee injury.

Our fantasy take: Prepare an alternative for Smith. Dontayvion Wicks and Makai Lemon could see additional targets if he sits, although extra opportunity does not guarantee a productive outing.
Jefferson has missed consecutive practices with an ankle injury ahead of Minnesota’s matchup with Miami. He has not been ruled out in the latest news available.

Our fantasy take: Jordan Addison becomes more interesting if Jefferson is unavailable. Wait for Minnesota’s final report before treating that increased role as certain.

Ladd McConkey’s Foot Injury Adds Concern
McConkey went from limited participation Wednesday to missing Thursday’s practice. His availability against Seattle remains uncertain.

Our fantasy take: Quentin Johnston and Tre’ Harris would have a clearer path to targets if McConkey cannot play. Keep replacement options ready rather than assuming McConkey will be available.

D’Andre Swift’s Status Could Open a Backfield Opportunity
Swift missed Thursday’s practice with a knee injury after not appearing on Wednesday’s report. The latest update does not establish whether he will miss Chicago’s game against the Jets.
Our fantasy take: Kyle Monangai is worth checking on waivers. If Swift is ruled out, Monangai could become a useful starting option because of the potential workload. Until then, he is a conditional pickup.
J
aylen Wright Improves While Seattle’s Backfield Remains Unsettled
Wright returned to full practice Thursday after missing Week 3. Miami lists him first on its depth chart, but Ollie Gordon II’s involvement means a shared workload remains possible.

Our fantasy take: Wright is worth rostering where available. His depth-chart position improves his outlook, but managers should avoid assuming he will receive every meaningful touch against Minnesota.
In Seattle, Jadarian Price missed Thursday’s practice with a chest injury. Emanuel Wilson could gain work if Price sits. Zach Charbonnet has returned to practice but remains ineligible to play in Week 4.

Our fantasy take: Wilson is a short-term option to monitor. Charbonnet is a potential future stash, not a replacement for this weekend.

Friday’s Reports Will Shape the Final Decisions
Zay Flowers returned to full practice Thursday, an encouraging development for Baltimore. Nico Collins has also resumed practicing, but his availability still needs confirmation.
Check final injury designations and game-day inactives before locking lineups. The biggest potential opportunities are Monangai if Swift sits, Addison if Jefferson sits, and Wicks or Lemon if Smith is unavailable.
